Blepharoplasty, more commonly known as eyelid surgery, is an outpatient procedure that refreshes the eye area by removing excess skin and fat deposits. Blepharoplasty can be performed on the upper eyelids, lower eyelids, or both. Surgery performed on the upper eyelids involves removing skin that droops or sags over the lids, sometimes into the line of sight; lower eyelid surgery treats the pockets of fat that accumulate under the eyes into “bags.”
Ideal blepharoplasty candidates are in good general health, with realistic expectations of the surgery. Patients with diabetes, thyroid disease, insufficient tear production, high blood pressure, circulatory disorders, or cardiovascular disease typically do not make strong blepharoplasty candidates.
Surgery usually takes one to two hours, depending on if you are having all four eyelids done. Local anesthesia with sedation is typically used in blepharoplasty procedures. You will be awake during surgery, but unable to feel any pain or discomfort.
Upper Eyelid Surgery
In upper eyelid surgery, the surgeon makes incisions in the crease of the upper eyelid, following the natural line of your eyelid. The placement of the incisions minimizes any scarring. Through the incisions, the surgeon separates the skin from the underlying fatty tissue and muscle, and removes excess skin and tissue that cause the eyelids to look droopy or inflated. Upper eyelid surgery can also deepen the crease in the upper eyelid, and reduce skin tissue above the eye that is impeding the peripheral vision. The incisions are closed using small sutures.
Lower Eyelid Surgery
Lower eyelid surgery removes pockets of fat underneath the eyes that are causing puffiness or under eye bags. The surgeon makes incisions below the lower eyelashes, and removes excess skin, fat, and if necessary, muscle. In certain cases, the surgeon may redistribute the fat to minimize puffiness and uneven swelling. If patients have pockets of fat below the lids but do not need any skin removed, the surgeon may elect to perform a transconjunctival blepharoplasty, where the incisions are made inside the lower eyelid. Transconjunctival blepharoplasty surgery is generally performed on patients with thicker skin with more elasticity.
Benefits of Blepharoplasty
Eyelid surgery dramatically improves the appearance of the eyes, and can improve the vision of patients whose lids droop into their line of sight. The surgery is fairly simple and quick, and results are natural looking and typically long-lasting. An added financial benefit is that for some, insurance may cover the procedure; this occurs in cases where blepharoplasty is performed to improve vision that has been affected by folds of excess eyelid skin.
Blepharoplasty can be combined with other surgical or injectable treatments to reduce other signs of aging. For example, dermal fillers may be injected into the eye area to fill in crow's feet. Or eyelid surgery may be combined with brow lift surgery to elevate a sagging brow line.
